New Service Request

As a new resident, we would like to start by welcoming you to the area. Our mission is to provide all our customers with the best possible service. This page was designed to give you all the information needed to quickly and easily set up your Water and Sewer account with CWSD.

This page and information is specifically for locations that have previously had service at the location. If this is a brand new home or account, please call the office for further details. Additional fees will apply.

Below are the steps necessary for setting up new accounts:

  • Commercial and Fire Hydrant Account: Must be set up at the office.

  • Residential accounts: Please complete and return a New User Agreement. This form can be hand delivered to the office, faxed to us or scanned and emailed back. Please be sure and review page two of the agreement.

  • Along with this New User Agreement, there is a $150 refundable deposit required to set up all new account.

    • You can hand deliver it to the office with your other paperwork or mail a check or money order made out to "CWSD" along with the agreement.

    • If you choose to fax or email the application back to us, once we receive it, we will call you to arrange for payment of your deposit. If you have not received a phone call from CWSD within the first business day from the time of submitting the application, please call the office for confirmation. Water will not be turned on until the deposit has been received.

  • In the left hand column of this page are some other useful forms for new customers. Please take a few minutes and review CWSD's Customer Service Policy. It is loaded with helpful information and will answer many of your questions.

  • We are on the West side of Centerton.

  • Take Arkansas Highway 102 west and watch for the water tower on the North side of the road and our sign.

  • Turn North on Keller Rd. and we are about 1/2 mile on the right.
